11/4/2023 EnchantaVaughn Approved Behavior: Upon arrival, client appeared to be in a positive mood. QMHP-C engaged mother in a discussion to determine her behaviors since the last session. Self-report, client verbalized to QMHP-C on how she has been struggling with resolving conflict within the last 2-3 days with her peers in the school setting and when she comes home to tell her mother, her and her mother seems to get into a verbal altercation. Client verbalized to QMHP-C her behaviors will last for at least 5 hours while in the school setting. She reports it is best for her to just be quiet so she doesn't get in trouble. Intervention: QMHP-C was able to get each family member to communicate with a clam voice tone, by issuing rules before talking. QMHP-C reminded the family that the rules consist of allowing each family member to speak without interruption. QMHP-C assisted Client how to identify more effective ways to improve self-esteem. QMHP-C assisted the parent on how to specifically define and identify problem behaviors, identify their reactions to the behavior, determine whether the reaction encourages or discourages the behaviors, and generate alternatives to the problem behavior. QMHP-C assisted the parent on how to implement key parenting practices consistently, including establishing realistic age appropriate rules for acceptable and unacceptable behavior, prompting of positive reinforcement to encourage more positive behavior. QMHP-C observed Client in the home to determine if she required assistance. QMHP-C observed Client complying with home rules and expectations. QMHP-C talked to Client about her confidence levels and how they can affect her thoughts and behaviors. QMHP-C encouraged Client to utilize positive self-talk as a means of decreasing anxiety and managing frustration when encountering difficult or challenging circumstances. Response: Client admitted to QMHP-C that she does sometimes find difficulty in adapting to the rules and regulations in the home only because she sometimes has to adjust to the schedule that mother has implemented. Client understands that her mother is trying to aid her in prioritizing skills and create structure in the home. Client mentioned that she has noticed mother has been making efforts on breaking the communication barrier in the home. Client also verbalized that her mother has also been consistent with her activeness in communicating with her and supporting her during this new transition. Client stated that mother has been praising and encouraging her when she displays positive and respectful behaviors. Client was able to reduce physical overactivity and become more attentive in the home. Client implemented appropriate social interaction skills.
